Two Surgeries, One Future: A Family’s Triumph Over NTDs

For over 30 years, Rosemary lived with the pain of trachomatous trichiasis (TT), a neglected tropical disease (NTD) that caused her eyelids to turn inward, causing her eyelashes to scrape against her eyes with every blink.

Triggered by repeated trachoma infections, TT can lead to irreversible blindness. For Rosemary, the discomfort became part of daily life. 

Despite the pain, she kept working, running a small tailoring business in Fukayosi Village in Tanzania’s Pwani region. But her condition made even simple tasks difficult, and she had no access to lasting treatment.

Tito’s Struggle

Rosemary’s grandson Tito was born with congenital hydrocele, a painful swelling in the scrotum. Cold, rainy days were the worst; Tito would stay home from school, missing out on his valuable education, unable to bear the discomfort. Rosemary had tried to get him treated as a baby, but doctors advised waiting until he turned five. When the time came, the cost of surgery was beyond reach. 

A Grandmother’s Advocacy

In 2023, Rosemary heard about a free medical camp at the district hospital. Determined to get Tito treatment, she attended to advocate for him. The medical camp was part of a broader strategy to tackle multiple health challenges simultaneously. This is the essence of Tanzania’s integrated health system approach: treating people, not just conditions. 

Initially, Tito’s case was denied as it didn’t meet the criteria for hydrocele caused by lymphatic filariasis, the camp’s focus. But Rosemary persisted and her determination paid off. The medical team accepted Tito’s case and performed a successful hydrocelectomy.

Two Surgeries, One Transformation

Tito’s surgery gave him a chance to be a child again, going back to school, back to soccer and importantly, continuing his education. But the story didn’t end there. 

During post-surgery follow-up, the medical team mentioned they also screened for eye complications.

“I went in to be checked and they saw that I had TT. I didn’t resist. I went in for surgery as soon as I could.” Rosemary said.

Back to Business, Back to School

After her recovery, Rosemary returned to her tailoring business and has continued supporting Tito’s education and his passion for football. To others living with conditions like TT, Rosemary has a clear message: 

“Go and get help. The operation is safe, it won’t kill or cripple you. It heals you.” 

Tanzania’s health system is evolving to meet the needs of its people more holistically. By integrating NTD programmes with other disease interventions through district hospitals, community health workers, and outreach programmes, the country is building a resilient system that reaches the most vulnerable. Since adopting the integrated approach, Tanzania has reduced TT prevalence to less than 5% in most districts and expanded access to hydrocele surgeries through district-level interventions.

Rosemary and Tito’s story is a powerful testament to the effectiveness of strengthened and integrated health systems, demonstrating how one act of advocacy can bring healing across generations. With continued investment in neglected tropical disease elimination, countless more families like Rosemary’s can reclaim their health, dignity, and future. 

To build on this progress, we must ensure sustained commitment from governments, donors, and partners. By investing in these vital programmes, we not only transform individual lives but also strengthen communities and economies.
